When considering advanced care planning, there are several directives that I would consider. Firstly, I would create a living will that outlines the types of medical procedures and treatments I do or do not wish to receive if my physical and/or mental capabilities do not allow me to make those decisions myself. This document can also include instructions regarding tissue donation as well as organ donation in order to ensure all wishes are respected upon passing.

Secondly, I would choose a healthcare proxy who is given the power of attorney to step into my shoes and make any necessary medical decisions on my behalf should I become incapacitated due to injury or illness. This person should be someone with whom I have an open line of communication so they understand what type of care or treatment path is aligned with my values prior to being granted this responsibility (Gill et al., 2020).

I am fortunate enough to have two people in mind for this task: My mother who has been by my side through thick and thin over the years, and one close friend whom I trust implicitly. Both individuals possess wisdom beyond their years as well understanding of me on an intimate level making them ideal candidates for assuming such responsibilities (Aldridge & McMullin 2019). They both share similar views on life which allows me peace of mind knowing whatever decision they may face will be made with good intent.

In conclusion, creating a living will is essential when it comes time for end-of-life considerations as it ensures that choices about one’s own care are respected regardless of their current state. Selecting a healthcare proxy further solidifies these choices by providing someone else the ability to make important medical decisions on your behalf if needed ensuring your wishes set forth in the living will are carried out accordingly.
