Maintaining a therapeutic alliance with clients who have co-occurring disorders (COD) is important, and difficult. Counselors should use primarily a supportive, empathic, and culturally appropriate approach when working with clients with COD. With some clients who have COD, it is important to distinguish behaviors and beliefs that are cultural in origin from those indicative of a mental disorder. COD are treated in a stage-wise fashion with different services provided at different stages. No matter whether the addiction came first or the mental illness was the first to arrive, treatment can make a big difference.
