Conducting a Community Health Needs Assessment (CHNA) involves defining the scope, collecting data, analyzing the data, determining key findings (Nursing Diagnosis), setting priorities and creating an action plan (Community Plan of Care), and sharing/disseminating findings.
To begin with, it is important to define the scope of the CHNA. This involves understanding what factors need to be taken into consideration when assessing the health needs of this particular community such as population size, type of services needed etc. Once that has been established it becomes easier to decide on which sources will provide most relevant information pertaining to local demographic and health conditions within area(CDC 2020).
Afterwards one can then proceed with collecting relevant data from both primary sources like surveys or interviews conducted directly from people living within community or secondary sources like public health records published by state government agencies (CDC 2020). This step must be undertaken carefully so that only reliable statistics are collected for further analysis.
Once all information has been collected one can then start analyzing it .By looking at patterns within gathered data ,one can determine which areas require most attention based on severity or prevalence rates associated them(Saunders et al., 2016). In addition any gaps present in care provision should also be identified during process.
Finally, once all necessary details have been assessed , practitioner may proceed developing nursing diagnosis outlining specific issues uncovered during evaluation along with prescribed interventions required address those challenges followed by drafting a detailed Community Plan Of Care addressing each problem identified earlier (Gordon & Reynolds 2018 ). Practitioner must also make sure that final plan put forward validates existing resources while creatively finding ways utilize them more effectively towards achieving desired outcomes. At the end practitioners should then share their findings widely across local stakeholders who maintain strong relationships potentially affected individuals so they can work together tackle current problems being experienced Houston’s local population.
