RECOMMENDING AN EVIDENCE-BASED PRACTICE CHANGE

The collection of evidence is an activity that occurs with an endgame in mind. For example, law enforcement professionals collect evidence to support a decision to charge those accused of criminal activity. Similarly, evidence-based healthcare practitioners collect evidence to support decisions in pursuit of specific healthcare outcomes.
In this Assignment, you will identify an issue or opportunity for change within your healthcare organization and propose an idea for a change in practice supported by an EBP approach.

• Reflect on the four peer-reviewed articles you critically appraised in Module 4, related to your clinical topic of interest and PICOT.
• Reflect on your current healthcare organization and think about potential opportunities for evidence-based change, using your topic of interest and PICOT as the basis for your reflection.
• Consider the best method of disseminating the results of your presentation to an audience.

Part 4: Recommending an Evidence-Based Practice Change
Create an 8- to 9-slide narrated PowerPoint presentation in which you do the following:
• Briefly describe your healthcare organization, including its culture and readiness for change. (You may opt to keep various elements of this anonymous, such as your company name.)
• Describe the current problem or opportunity for change. Include in this description the circumstances surrounding the need for change, the scope of the issue, the stakeholders involved, and the risks associated with change implementation in general.
• Propose an evidence-based idea for a change in practice using an EBP approach to decision making. Note that you may find further research needs to be conducted if sufficient evidence is not discovered.
• Describe your plan for knowledge transfer of this change, including knowledge creation, dissemination, and organizational adoption and implementation.
• Explain how you would disseminate the results of your project to an audience. Provide a rationale for why you selected this dissemination strategy.
• Describe the measurable outcomes you hope to achieve with the implementation of this evidence-based change.
• Be sure to provide APA citations of the supporting evidence-based peer reviewed articles you selected to support your thinking.
• Add a lessons learned section that includes the following:
o A summary of the critical appraisal of the peer-reviewed articles you previously submitted
o An explanation about what you learned from completing the Evaluation Table within the Critical Appraisal Tool Worksheet Template (1-3 slides)

Part 4: Recommending an Evidence-Based Practice Change

Slide 1: Title Slide

Slide 2: Healthcare Organization Description

[Include a brief description of your healthcare organization, including its culture and readiness for change. You may opt to keep various elements of this anonymous, such as your company name.]

Slide 3: Problem or Opportunity for Change

[Describe the current problem or opportunity for change. Include in this description the circumstances surrounding the need for change, the scope of the issue, the stakeholders involved, and the risks associated with change implementation in general.]

Problem:

  • High rates of readmission among patients with chronic heart failure
  • Scope of the issue:
    • Heart failure is the leading cause of hospitalization and readmission among Medicare beneficiaries
    • Readmissions are costly and can lead to worse health outcomes for patients
  • Stakeholders involved:
    • Patients and their families
    • Healthcare providers
    • Payers
    • Policymakers
  • Risks associated with change implementation:
    • Resistance from healthcare providers
    • Financial costs
    • Unintended consequences

Slide 4: Proposed Evidence-Based Practice Change

[Propose an evidence-based idea for a change in practice using an EBP approach to decision making. Note that you may find further research needs to be conducted if sufficient evidence is not discovered.]

Proposed change:

  • Implement a telemonitoring program for patients with chronic heart failure
  • Telemonitoring is a remote monitoring system that allows healthcare providers to track patients’ vital signs and other health data from home
  • Evidence suggests that telemonitoring can reduce readmissions among patients with chronic heart failure

Slide 5: Plan for Knowledge Transfer

[Describe your plan for knowledge transfer of this change, including knowledge creation, dissemination, and organizational adoption and implementation.]

Knowledge creation:

  • Conduct a literature review to identify the best practices for implementing a telemonitoring program for patients with chronic heart failure
  • Develop a training program for healthcare providers on how to use the telemonitoring system

Knowledge dissemination:

  • Present the findings of the literature review to the organization’s leadership team
  • Hold educational sessions for healthcare providers on the telemonitoring program

Organizational adoption and implementation:

  • Develop a plan for implementing the telemonitoring program, including timelines, resources, and evaluation criteria
  • Work with the organization’s IT department to integrate the telemonitoring system into the organization’s electronic health record system

Slide 6: Dissemination of Results

[Explain how you would disseminate the results of your project to an audience. Provide a rationale for why you selected this dissemination strategy.]

Dissemination strategy:

  • Publish a paper in a peer-reviewed journal
  • Present the findings of the project at a national healthcare conference

Rationale:

  • Publishing the findings of the project in a peer-reviewed journal will allow the research to be disseminated to a wide audience of healthcare professionals and researchers
  • Presenting the findings of the project at a national healthcare conference will allow the research to be disseminated to a more targeted audience of healthcare professionals who are interested in implementing telemonitoring programs for patients with chronic heart failure

Slide 7: Measurable Outcomes

[Describe the measurable outcomes you hope to achieve with the implementation of this evidence-based change.]

Measurable outcomes:

  • Reduction in readmission rates among patients with chronic heart failure
  • Improvement in patient satisfaction
  • Reduction in healthcare costs

Slide 8: Lessons Learned

[Include a summary of the critical appraisal of the peer-reviewed articles you previously submitted and an explanation about what you learned from completing the Evaluation Table within the Critical Appraisal Tool Worksheet Template.]

Summary of critical appraisal:

  • All four peer-reviewed articles were well-written and used rigorous methodology
  • The articles provided strong evidence that telemonitoring can reduce readmissions among patients with chronic heart failure

Lessons learned from completing the Evaluation Table:

  • It is important to critically appraise the evidence before making decisions about implementing evidence-based practice changes
  • The Evaluation Table is a useful tool for evaluating the quality of evidence-based research

Slide 9: References

[Include a list of all references used in the presentation.]

Conclusion

This presentation has described an evidence-based practice change that could be implemented in a healthcare organization to reduce readmissions among patients with chronic heart failure
