I recall a situation during my clinical practice in a fast-paced emergency department where a junior nurse hesitated to clarify a medication order because a senior nurse had a reputation for being condescending. This hesitation could have led to a medication error. If psychological safety had been present, the junior nurse would have felt comfortable seeking clarification, preventing potential harm.
