In this written assignment, you will conduct a community assessment that incorporates a windshield survey and apply the nursing process to your findings. Develop a three-page (must be three full pages of content) APA formatted paper using current (five years and less to date) peer-reviewed references. Three references are required. Assessment (Collect at least two elements of demographic data that support the community health nursing diagnosis. Resources are provided within the Success Platform within Week 2 Assignment Instructions.)
Diagnosis (Identify two priority nursing diagnoses and one community health promotion diagnosis. Be sure to identify the target population and level of prevention.
Plan (Include the community or public health nurse’s role in the development and implementation of the interventions for each nursing diagnosis.)
Intervention (Provide details about the intervention (s) and any community stakeholders necessary for carrying out the intervention.)
Evaluation (Provide a summary of an evaluation plan that includes goals and criteria to evaluate the effectiveness of the intervention(s).)

Assessment:
The city of Los Angeles is an urban area that has a population of approximately 4 million people, with the majority being Latinos (49.9%) and African-Americans (10.6%). The median household income for this population is $52,400 with 31% living below the poverty line (U.S. Census Bureau, 2019). In terms of health care access, 24% lack basic health insurance coverage while 21% indicated they were unable to obtain medical care in the past year due to cost (Los Angeles County Department of Public Health [LACDPH], 2019). Additionally, the LACDPH reported that there are several “hot spots” within LA where residents have higher rates of morbidity due to long-term exposure to environmental factors such as air pollution and toxins from industrial sites or power plants (LACDPH, 2019).
Diagnosis:
Based on these findings it appears that two priority nursing diagnoses are appropriate for this community; 1) Risk for Injury related to environmental hazards among adults aged 18-64 years old 2) Risk for inadequate nutrition related to financial barriers among low-income families with children under 18 years old.
The target population for both diagnoses would include those individuals who are at risk for injury or inadequate nutrition due to their social economic status or environmental exposures respectively . Furthermore , interventions would be aimed at primary prevention as these issues can be addressed before development of any associated complications occur .
In addition, one community health promotion diagnosis should also be identified which could focus on increasing physical activity and promoting healthy eating habits among youth aged 10 – 14 years old in order to reduce prevalence of obesity in this demographic group(Li et al., 2020 ). This diagnosis should target secondary prevention since it will aim at reducing morbidity by decreasing risk factors such as poor diet and lack of exercise amongst high risk groups.
