1. Develop a plan to address the identified health concern or problem.

a. Provide an introduction to identified health concern/problem. This may be based on your community assessment project. What is the identified health concern? Why? What is the community setting and population? AIRRA
You can use the same topic you did your teaching project on. For example, “Overtown is a neighborhood in Miami with a crime rate XYZ% higher than the national average. There are few resources available to this community and the youth that live there. We chose gun safety for youth living in Overtown in order to combat some of the accidental deaths that occur. In addition, we are also proposing a gun buyback program to reduce the number of homicides.

b. Provide supportive data from assessment of community, including at least two reputable statistical references from community to support your identified concern. (MILARGO)
As previously described, the crime rate in Overtown is higher than the national average. It has been shown that XYZ deaths occur in Overtown due to illegal guns in the hands of minors. This figure is higher than the national average of XYZ. The number of homicides also exceeds the national average…etc.

c. Describe your plan/project you did to assist with the health concern/problem for your community. (Include pictures of your group in action) (CLAUDIA)
Describe gun safety education plan (already done in teaching project). Describe any other plans, such as the gun buyback program: He will host a gun buyback program at the park on January 10, etc.

d. Include at least four literature reviews to support the plan you are developing which is current (within the last five years). (ALL, this is the references)
These can include previous literature reviews but try to include at least one new one and make sure it supports your mission.

e. Describe how you will plan on evaluating this plan in measurable goals. ALEX
The pre and posttest results for students in the education program will be evaluated. A follow up questionnaire one month later will be used to evaluate knowledge retention. Collecting at least 10 unregistered guns is estimated to reduce crime rate by XYZ%. Will evaluate the homicide rate and accidental gun death rate throughout the year following the program to determine efficacy. ALEX

f. Design and print applicable materials, handouts, etc. needed for your plan. (POWERPOINT)
This can include advertisements for the buyback program and materials for gun safety teaching.

a. Provide at least two references either a learning theory or a health promotion model (Example: Pender’s Health Promotion Model) for this material or handout that best fits your community and described how you used this theory in your project.

Will reduce illegal guns on the street to reduce homicides and will reduce accidental deaths in children.

3. Involve any appropriate community agency that has a similar interest in the problem (MJ)
a. Describe your agency.
We chose to work with the Overtown Youth Center as they serve a large portion of Overtown’s youth. They can incorporate this program into their after-school activities and engage their sponsors (i.e. Alonso Mourning) into the initiative.

b. How are they meeting/or not meeting the needs of the community you have identified the health concern or problem? Gun safety is not currently being addressed at the youth center. Incorporating this program into the center aligns with their mission of XYZ.

c. How did you assist them?
Provided the gun safety package, provided teaching to the staff, led a gun safety awareness day activity with the center to kick off the initiative.

I spoke with John Smith, the curriculum coordinator at the youth center (find the actual person’s name).

4. Prepare to present a 3-minute presentation to a governing board about your concern such as a city council, school board, etc. (ALL)
a. Plan to present this to the class as if you were planning to present to a governing board, etc. as time may not permit actual presentation.

b. Submit this presentation or per the directions of your instructor. (ALL)
This must include a financial analysis. Explain why it is a problem, the benefits of fixing it, describe your plan, and how you plan to finance it. This is the time to prove to the board that the funds you are requesting are substantiated and will truly benefit the community.

5. This community action plan should or may also incorporate the Community Teaching Assignment.

6. Ideas for Community Action Plans may include:
a. Development of a program plan to address a concern
b. Plan and participate in a community health fair
c. Plan and participate in a community vaccination project
d. Develop a promotional event for your agency related to your concern i.e. sock drive for homeless children, etc.
e. Develop a fundraiser for a community agency that addresses your concern
f. Develop a resource book related to your community problem
