Sample Solution
In order to address the patient, family, or population problem I have defined – namely the lack of access to accurate and up-to-date health information among low-income communities – healthcare technology, coordination of care, and community resources can be applied. Healthcare technology such as electronic medical records (EMRs) should be utilized by providers in order to store and share valuable medical data with other members of the healthcare team quickly and accurately. This could also include leveraging telemedicine services which allow for remote clinical consultations between doctors and patients thus eliminating any need for physical visits that may present a financial burden (Chen et al., 2020).
Coordination of care is another important factor when looking to improve access to health information among low income populations. By designing collaborative networks between hospitals, clinics ,and other healthcare facilities providers can ensure that all necessary data pertaining to a particular patient’s condition is shared in a timely manner preventing misdiagnoses or unnecessary treatments while also minimizing costs associated with transferring paper records.(Umscheid et al., 2011).
Lastly, it would be important utilize existing local community resources such as churches, libraries, schools etc. not only as places where patients can receive basic health education but also sites where they can register for personal health assistance programs like Medicaid or Medicare if they qualify. Additionally providing accessible computer stations at these locations – along with assistance from staff/volunteers -can help bridge the “digital divide” i.e.- disparity between those who do and don’t have internet access – enabling them better understand their diagnoses online or even submit applications for financial aid programs directly.
