Learning Plan
This assessment is about what I expect to learn from the placement. The organisation Permalink Support Services helps people with disability, they provide employment for people with special needs. For this assessment I need to write about what I look forward when it comes to interacting, how to approach people with disability and in return how I can help them as a case manager.
Sample Solution
When interacting with people who have a disability, I look forward to creating an environment of trust and understanding. By viewing the individual first, rather than their diagnosis or challenges, I strive to communicate in a respectful and empowering manner. As a case manager my role is to advocate for each client’s rights as well as help them achieve their fullest potential. To do this it is important that we establish effective communication strategies such as active listening and providing accurate information about available resources. In addition, I am committed to working collaboratively with other professionals involved in the individual’s care plan so that together we can ensure quality services are provided within an atmosphere of dignity and respect. Through these interactions my goal is always to empower individuals by combining compassion with advocacy while promoting independence and self-determination towards achieving their desired outcomes.
