Determine how health care technology, coordination of care, and community resources can be applied to address the patient, family, or population problem you’ve defined. In addition, plan to spend approximately 2 direct practicum hours exploring these aspects of the problem with the patient, family, or group you’ve chosen to work with and, if desired, consulting with subject matter and industry experts. Document the time spent (your practicum hours) with these individuals or group in the Academic Portal Volunteer Experience Form. Report on your experiences during the second 2 hours of your practicum.
Introduction
As a baccalaureate-prepared nurse, you’ll be positioned to maximize the use of technology to achieve positive patient outcomes and improve organizational effectiveness. Providing holistic coordination of patient care across the entire health care continuum and leveraging community resource services can lead both to positive patient outcomes and to organizational improvements.
Sample Solution
In order to address the patient, family, or population problem I have chosen to work with, I plan to spend approximately 2 direct practicum hours exploring how health care technology, coordination of care, and community resources can be applied. During this time, I will focus on educating my patients/families/group members about the various types of health care technologies available such as electronic medical records (EMR) systems, telemedicine services, online patient portals for accessing medical information and prescription refills. Additionally, I will discuss the importance of coordinating their care among different providers in order to ensure they are receiving comprehensive and adequate treatment. Finally, I will provide them with contact information for local community resources that can help support them in achieving their goals such as primary care centers or social workers who can assist them with connecting to financial assistance programs.
I am looking forward to learning more about these topics through both direct interaction with my patients/family members/group members and consulting with subject matter experts during these two hours. As part of documenting my experience during this time in the Academic Portal Volunteer Experience Form, I am hoping that by spending these two practicum hours exploring how health care technology coordination of care and community resources can be applied effectively it will be beneficial not only for those whom I interact with but also for myself as a provider so that when treating similar cases in the future I already have some insight into what interventions may work best based off past experiences like this one.
