Understanding family structure and style is essential to patient and family care. Conducting a family interview and needs assessment gathers information to identify strengths, as well as potential barriers to health. This information ultimately helps develop family-centered strategies for support and guidance.

This family health assessment is a two-part assignment. The information you gather in this initial assignment will be utilized for the second assignment in Topic 3.

Develop an interview questionnaire to be used in a family-focused functional assessment. The questionnaire must include three open-ended, family-focused questions to assess functional health patterns for each of the following:

  1. How does your family cope with stress and difficult situations?

  • What are your family’s goals for health?
  • What are your family’s resources and supports?

Interview Instructions

  • Be respectful of the family’s time and privacy.
  • Ask open-ended questions that allow the family to share their own perspectives.
  • Listen carefully to the family’s responses.
  • Summarize the family’s responses to ensure that you have understood them correctly.
  • Thank the family for their time and participation.

Interview Example

I recently conducted a family-focused functional assessment with a family of four. The family consisted of the parents, John and Mary, and their two children, ages 10 and 12. The interview was conducted in the family’s home.

Values/Health Perception

  • The family values health and well-being. They believe that it is important to eat healthy foods, get enough sleep, and exercise regularly.
  • The family’s values influence their health-related decisions. For example, they choose to eat organic foods and to avoid processed foods.
  • The family’s strengths related to health include their strong support for each other and their commitment to healthy living. Their challenges related to health include the father’s high blood pressure and the mother’s anxiety.

Nutrition

  • The family’s usual eating habits are fairly healthy. They eat a variety of fruits, vegetables, and whole grains. They also limit their intake of processed foods and sugary drinks.
  • The family’s favorite foods include grilled chicken, salmon, and brown rice.
  • The family’s challenges related to eating healthy include the father’s love of junk food and the mother’s busy schedule.

Sleep/Rest

  • The family’s usual sleep habits are good. They all go to bed at a reasonable hour and get enough sleep.
  • There are no challenges related to sleep that the family is facing.

Elimination

  • The family’s usual elimination habits are normal. They all have regular bowel movements and bladder function.
